Financial Agreement Policy

Payment Requirements
  • Full Payment: Payment for services is required at the time of the appointment unless prior arrangements are made.
  • Accepted Methods: We accept cash, credit/debit cards, and payment plans through offered and approved finance options.
Insurance and Coverage
  • Insurance: We do not accept insurance. Payment is due in full at the time of service.
Refunds and Cancellations
  • Service Refunds: No refunds will be issued for services rendered. If a service is not performed as expected, please contact us within 48 hours to discuss a resolution.
  • Product Refunds: Please note that all product sales are final and we do not offer refunds. We are happy to assist with any questions or concerns you may have to ensure you’re satisfied with your purchase.
  • Cancellations: Cancellations must be made at least 24 hours in advance. Late cancellations or no-shows will incur a charge of $75 for a loss in schedule time.
Pre-Authorized Payments
  • Deposits: For certain treatments, a deposit may be required. This deposit is non-refundable but can be applied to future services if cancellation occurs within the policy timeframe.
Payment Plans
  • Availability: Payment plans are available upon request and subject to approval. Terms and conditions apply.
No Guarantee of Outcomes
  • Outcomes: While we strive to achieve the best possible results, we do not guarantee specific outcomes. Any expenses incurred by medical or aesthetic care you elect to receive outside of the office, such as, but not limited to, dissatisfaction of treatment outcomes or adverse events, will be the sole responsibility of the patient.
Dispute Resolution
  • Resolution: Any disputes regarding charges or services should be addressed directly with our management team. We are committed to resolving issues amicably.

No Guarantee of Outcome Policy

At Sculpt Aesthetics, we are dedicated to providing high-quality services and achieving the best possible results for our clients. However, it is important to understand the following:

Individual Results Vary
  • Personal Factors: The effectiveness and results of any aesthetic treatment can vary significantly from person to person. Factors such as individual skin type, health conditions, and lifestyle may influence the outcome.
  • No Guarantee: We do not guarantee specific results or outcomes from our treatments. Any expected results are based on general expectations and are not guaranteed.
Consultation and Expectations
  • Pre-Treatment Consultation: We encourage thorough consultation before any treatment to discuss potential results and manage expectations. Our practitioners will provide guidance based on their professional experience, but individual results cannot be assured.
  • Informed Decision: By proceeding with treatments, clients acknowledge understanding that outcomes may differ from anticipated results and agree to the inherent risks associated with the procedure.
Post-Treatment Care
  • Patient Responsibility: Optimal results often require adherence to post-treatment care instructions provided by our practitioners. It is the client’s responsibility to follow these guidelines to support the best possible outcome.
Additional Services
  • Additional Treatments: Should additional treatments be required to address concerns or achieve desired results, these will be at the client’s expense. The cost of any additional services or corrective measures outside of our facility is the client’s responsibility.
Feedback and Resolution
  • Feedback: If you have concerns about your treatment, please contact us directly. We are committed to addressing your concerns and exploring possible solutions. However, we do not offer refunds or compensation based on results or dissatisfaction.

By receiving services from Sculpt Aesthetics, you acknowledge and accept that we cannot guarantee specific outcomes and agree to the terms of this policy.

Cancellation and Late/No-Show Policy

Cancellation Policy
  • Notice Requirement: To avoid a cancellation fee, please provide at least a 24-hour notice if you need to cancel or reschedule your appointment.
  • Fee for Late Cancellation: Cancellations made with less than a 24-hour notice will incur a $75 fee.
No-Show Policy
  • No-Show Fee: Failure to attend a scheduled appointment without prior notice will result in a $75 fee.
Late Arrival Policy
  • Arrival Time: Please arrive on time for your appointment. If you haven’t filled out the necessary intake forms, please arrive 10 minutes early to complete them prior to your appointment time. If you arrive late and there is insufficient time remaining to perform the service, as determined by the provider, you will be charged the $75 cancellation/late fee.
  • Service Adjustment: If late arrival impacts the ability to complete the scheduled service, the provider may adjust or reschedule the appointment.
Fee Payment
  • Payment: Fees for late cancellations, no-shows, or late arrivals will be charged to your account and must be settled before booking future appointments.

Right of Refusal Policy

Right of Refusal
  • Service Refusal: Sculpt Aesthetics reserves the right to refuse service to any individual who appears to be under the influence of drugs or alcohol, or who is not of sound mind and able to make fully informed decisions about their treatment.
Consent and Decision-Making
  • Advised State: It is advised that patients refrain from signing consent forms or undergoing any treatments while under the influence of substances that could impair their ability to make sound decisions.
  • Patient Responsibility: It is the patient’s responsibility to ensure they are in a clear state of mind when consenting to treatments. Sculpt Aesthetics assumes that patients are making informed and responsible decisions regarding their care.
Provider's Role
  • Assumption of Fitness: While our providers will assess the suitability of the patient for treatment, Sculpt Aesthetics is not responsible for determining the patient’s state of mind or ability to make decisions beyond their observations. We rely on patients to disclose any factors that may affect their decision-making.

By receiving services from Sculpt Aesthetics, you acknowledge and accept our right to refuse service and the importance of being in an appropriate state of mind for making informed decisions.

Age Policy

FDA Approval and Age Restrictions
  • FDA-Approved Age Restrictions: Some treatments are not FDA-approved for certain ages. Sculpt Aesthetics adheres to these guidelines but may offer “off-label” treatments at our discretion.
Off-Label Use
  • Off-Label Treatments: Some products or services may be used off-label. Sculpt Aesthetics may provide these treatments based on our clinical judgment and with the patient’s acknowledgment of the off-label nature of these services.
Minors and Parental Consent
  • Treatments for Minors: Certain treatments may be performed on minors with both patient and parental consent. These services will be provided at the sole discretion of the individual provider, who will assess the appropriateness based on the specific circumstances.
Patient Acknowledgment
  • Informed Consent: By receiving services at Sculpt Aesthetics, patients acknowledge understanding of the age-related restrictions and the potential off-label use of products or services. Patients also accept that minors may undergo treatments only with appropriate consent and provider discretion.
